What You'll Do

• Participate in Interdisciplinary Team (IDT) meetings to assess needs and develop personalized care plans.
• Evaluate functional abilities, strength, and mobility to design targeted therapy programs.
• Deliver therapeutic services including gait training, strength conditioning, and mobility restoration.
• Facilitate engaging individual and group exercises focused on rehabilitation and wellness.
• Provide care aligned with physician orders and individual treatment goals.
• Educate participants and their families to support safe and effective home recovery.
• Recommend follow-up care and referrals based on patient progress and needs.
• Conduct occasional in-home or facility-based therapy visits when necessary.
• Collaborate with nursing homes, assisted living, and SNF staff to oversee participants’ rehab journey.
• Maintain and manage therapy equipment for safe and efficient operations.

Schedule & Work Environment

• Hours : Monday–Friday, 8:00 AM – 5:00 PM (100% on-site)
• Travel : Minimal; occasional local travel to homes or care facilities
